International Breastfeeding Initiatives and their Relevance to the Current State of Breastfeeding in the United States

Introduction

Lactation is an ancient process that predates placental gestation. It represents the normal and expected way to feed infants and young children, yet continues to suffer from cultural and commercial barriers that make it difficult for mothers to adhere to the medical recommendation to breastfeed exclusively for 6 months, and to continue breastfeeding with appropriate complementary foods for 1 year and beyond.

Infant feeding through the ages has been subject to shifting attitudes toward the mother/child relationship, the understanding (or misunderstanding) of the process of lactation and the composition of human milk, and the escalating acceptance of the use of human milk substitutes.

Hospital Practices that Increase Breastfeeding Duration: Results from a Population-Based Study

ABSTRACT

Background: A high percentage (83%) of mothers in Colorado initiate breastfeeding; but in keeping with national breastfeeding trends, many of them discontinue breastfeeding within the first few months. The objective of this study was to determine the effects of hospital practices on breastfeeding duration and whether the effects differed based on maternal socioeconomic status.

Methods: Pregnancy Risk Assessment Monitoring System data were used to calculate breastfeeding duration rates for all Colorado mothers in 2002 to 2003. Breastfeeding duration rates were determined for recipients of each of nine hospital practices included in the survey compared with rates for nonrecipients.

Egyre több a bizonyíték arra, hogy ez a jelenség áttörést jelent a gyakran előforduló korai szoptatási problémák leküzdésében*.

Early skin-to-skin contact for mothers and their healthy newborn infants

Abstract

BACKGROUND: Mother-infant separation postbirth is common in Western culture. Early skin-to-skin contact (SSC) begins ideally at birth and involves placing the naked baby, covered across the back with a warm blanket, prone on the mother’s bare chest. According to mammalian neuroscience, the intimate contact inherent in this place (habitat) evokes neurobehaviors ensuring fulfillment of basic biological needs. This time may represent a psychophysiologically ‘sensitive period’ for programming future behavior.

OBJECTIVES: To assess the effects of early SSC on breastfeeding, behavior, and physiological adaptation in healthy mother-newborn dyads.

Szoptatás: az élet első órája

Korai mellre tétel és kizárólagos szoptatás az élet első fél évében – pusztán ezzel több mint egymillió kisbaba életét lehetne megmenteni

Tény, hogy egymillió kisbabát lehetne megmenteni,

  • ha világszerte lehetővé tennék azt az egyszerű dolgot, hogy a szülés utáni első órában szophasson minden újszülött;
  • ha elősegítenék, hogy az anya a meztelen kisbabát azonnal magához ölelhesse, vagyis bőrkontaktus jöhessen létre rögtön a szülés után, és hat hónapos korig minden baba csak anyatejen élne;
  • ha arra bátorítanák minden állam egészségügyi miniszterét, és más vezetőket, hogy a szülés utáni első órában történő szoptatást tekintsék a prevenciós egészségügy fő mutatójának;
  • ha a családok tudnák, hogy milyen fontos, hogy mi történik az újszülöttel élete első órájában, bátorítanák őket, hogy járjanak utána, hogy kisbabájuk megkapta-e azt, ami számára a legmegfelelőbb;
  • ha támogatnák a javított és újjáélesztett Bababarát Kórház Kezdeményezés térhódítását (Baby Friendly Hostpital – BFHI), kihangsúlyozva, hogy fontos lenne a jelen rendszerbe való beillesztése és kiterjesztése, kiemelve a korai mellre tétel jelentőségét.

Early lactation performance in primiparous and multiparous women in relation to different maternity home practices. A randomised trial in St. Petersburg

Abstract

BACKGROUND: There are not many studies exploring parity differences in early lactation performance and the results obtained are fairly often contradictory. The present study investigated the effect of different maternity home practices in St. Petersburg, Russia, as well as of physiological breast engorgement and maternal mood, on milk production in primi- and multiparous women on day four. The amount of milk was studied in relation to the duration of “nearly exclusive” breastfeeding.

METHODS: 176 mother-infant pairs were randomised into four groups according to an experimental two-factor design taking into account infant location and apparel.

Breastfeeding Initiation and Birth Setting Practices: A Review of the Literature

Abstract

A number of practices during the intrapartum and very early postnatal period may affect breastfeeding initiation. All women should be encouraged to breastfeed soon after birth, with extra attention paid to the identification and offer of extra support for those women at higher risk of not successfully initiating and continuing breastfeeding.

Factors that affect breastfeeding initiation include: implementation of the Baby-Friendly Hospital Initiative Ten Steps to Successful Breastfeeding at a service level; avoidance of the use of intramuscular narcotic analgesia, particularly near the end of the first stage of labour; not separating mothers and babies after birth for routine procedures; and placing healthy newborns on their mothers’ chest/abdomen (skin-to-skin) routinely.

Evidence Based Breast-Feeding Promotion: The Baby-Friendly Hospital Initiative

Abstract

The Baby-Friendly Hospital Initiative (BFHI) is the translational tool developed by WHO and UNICEF to promote breast-feeding (BF) in maternity wards worldwide. BFHI was officially launched in the 1980s based on a “common sense” approach. Since then, research conducted in Latin America has shown that BFHI is highly cost-effective.

BF trends over the past 2 decades strongly suggest that BFHI has had a global impact on BF outcomes. The 10th step of BFHI related to community-based BF promotion is one of the most challenging ones to address. Randomized controlled trials conducted in the Americas, Asia, and sub-Saharan Africa indicate that peer counseling is a very efficacious tool for increasing EBF rates.

A comparison of breastfeeding rates in an urban birth cohort among women delivering infants at hospitals that employ and do not employ lactation consultants.

Abstract

OBJECTIVE: To compare rates of breastfeeding at hospital discharge between facilities that employ and do not employ International Board Certified Lactation Consultants (IBCLCs).

METHODS: This study used a cross-sectional design. Data from 11,525 birth certificates of Philadelphia residents who delivered in 2003 were used. Breastfeeding was assessed using a question included on the Pennsylvania birth record, “Is the infant being breastfed at discharge?” The Philadelphia Department of Public Health’s lactation consultants collected information on number of hours worked annually by IBCLCs by facility.

RESULTS: After adjusting for race/ethnicity, education, insurance status, age, marital status, route of delivery, birth weight, and gestational age, delivering in a hospital that employed an IBCLC was associated with a 2.28 (95% confidence interval [CI] =1.98,2.62) times increase in the odds of breastfeeding at hospital discharge.

The Breastfeeding Support and Promotion in Baby-Friendly Maternity Hospitals and Not-as-Yet Baby-Friendly Hospitals in Russia

Abstract

Objective: The objective was to evaluate implementation of the WHO/UNICEF “Ten Steps to Successful Breastfeeding” as defined by the Baby-Friendly Hospital Initiative in eight maternity hospitals in the Moscow region. Four maternity hospitals had been certified Baby- Friendly Hospitals (BFHs), the experimental group; and four maternity hospitals Not-as-Yet Baby Friendly, the control group (NBFHs).

Methods: Maternal interviews and infant breastfeeding rates were the primary outcomes of the study. In total, 741 healthy postpartum women from the experimental and control group were interviewed: 383 and 358, respectively. Interviews were conducted over 5 months, from May to July 2004.
