Hogyan könnyítsük meg gyermekünk megérkezését e világra?

Első unokám, Sebastian 8 héttel hamarabb született és légzési nehézségei voltak. Napokig szinte minden idejét inkubátorban töltötte, lélegeztető gépre kapcsolva, miközben mi csak a műanyag falakon keresztül nézhettük őt vágyakozva arra, hogy végre megérinthessük és a kezünkben tarthassuk.

Édesanyját egy kissé szokatlan dologgal ajándékoztam meg, erre biztosan nem számított: több darab, elöl végig gombolós inggel. Abban a pillanatban, amikor Sebastian már nem szorult gépi lélegeztetésre, édesanyja megértette az ajándék lényegét. Az inge alá helyezte őt, bőrük összeért, miközben továbbra is a monitorokhoz volt kötve a pici.

Energy Expenditure for Breastfeeding and Bottle-Feeding Preterm Infants

Abstract

OBJECTIVE: We hypothesized that resting energy expenditure (REE) would be higher after breastfeeding than after bottle-feeding.

METHODS: Nineteen preterm infants (gestational age: 32 weeks) in stable condition who were nourished entirely with their mothers’ breast milk were assigned randomly to feeding either by bottle or at the breast. Each infant served as his or her own control subject. REE was measured for 20 minutes after feeding. Breast milk quantity was evaluated with prefeeding and postfeeding weighing. REE values for bottle-feeding and breastfeeding were compared with paired t tests.

Supporting Premature Infant Nutrition

Premature infants who receive human milk have the best outcomes; medically, nutritionally, and developmentally.

The Supporting Premature Infant Nutrition (SPIN) program was developed to address the challenges of helping mothers produce sufficient breastmilk for their premature infants, and to improve the manner in which Neonatal Intensive Care Units support optimal nutrition and growth in their most vulnerable population of patients.

Our Mission: To create a Center of Excellence in neonatal nutrition focused on the provision, analysis, and research of human milk to improve nutritional and neurodevelopmental outcomes in preterm babies.

This website has been developed to provide parents and NICU staff the tools they need to work together toward this goal.

Koraszülöttek anyatejes táplálását segítő honlap indult a napokban

A Kaliforniai Egyetem San Diegoi Egészségügyi Központja a napokban új honlapot indított útjára, aminek célja, hogy értékes ismereteket adjon át a családok és az egészségügyben dolgozók számára arról, hogyan lehet humán tejjel tápláni a koraszülött és kis súylú csecsemőket. A honlap létrejöttét a March of Dimes Alapítvány 10 000 dolláros adománya segítette.

„Honlapunk egyik célja, hogy segítse a többi kórházat abban, hogy adaptálja a mi anyatejes táplálási modellünket a saját újszülött intenzív osztályán” – nyilatkozta Dr. Jae Kim a Koraszülöttek Táplálását Támogató Program (SPIN) orvosigazgatója. „A program bevezetése óta az anyatejes táplálás 15%-kal nőtt. Örülnénk, ha más intézményekben is hasonló növekedés tanúi lehetnénk.”

Breastfeeding promotion for infants in neonatal units: a systematic review and economic analysis

Objectives: To evaluate the effectiveness and costeffectiveness of interventions that promote or inhibit breastfeeding or feeding with breastmilk for infants admitted to neonatal units, and to identify an agenda for future research.

Conclusions: Despite the limitations of the evidence base, kangaroo skin-to-skin contact, peer support, simultaneous breastmilk pumping, multidisciplinary staff training and the Baby Friendly accreditation of the associated maternity hospital have been shown to be effective, and skilled support from trained staff in hospital has been shown to be potentially cost-effective.
All these point to future research priorities.

Újfajta fejési technika a tejtermelés fokozására

A koraszülő édesanyák gyakran szembesülnek azzal, hogy gyermekük túl kicsi és erőtlen ahhoz, hogy szopjon. Ezért a tejtermelést fejéssel kell beindítani és fenntartani mindaddig, amíg a kicsi elég erős lesz ahhoz, hogy kizárólag édesanyja emlőjéből táplálkozzon.
Egy kaliforniai kutatás során, amelyben a Lucile Packard Gyermekkórház és a Stanford Egyetem Egészségügyi Főiskolája vett részt, újfajta, a gépi és a kézi fejés kombinációján alapuló fejési technikát dolgoztak ki a kutatók, amellyel az édesanyák jóval több tejet termeltek, mint a korábban alkalmazott technikákkal. A tanulmány a Journal of Perinatology c. folyóiratban jelent meg.

A tanulmányban 67 koraszülő édesanya vett részt, akiknek megtanították, hogyan alkalmazzák az elektromos mellszívó és a kézi fejés kombinációját tejük lefejésére.

Effects of Different CMV-Heat-Inactivation-Methods on Growth Factors in Human Breast Milk

Preterm infants can inoculate virulent cytomegalovirus (CMV) through their mothers’ raw breast milk. Complete virus inactivation is achieved only by heat treatment, but the effect on growth factors has never been assessed systematically.

Neither long- nor short-time heating methods changed the concentration of EGF. Only short heating methods (5 s, 62–72°C) can preserve, almost completely, the concentrations of IGFs in human milk, whereas Holder-Pasteurization does not.

Abstract

Urinary tract infection (UTI) differs between preterm and older infants and children in terms of prevalence, clinical presentation, causative organism, and rate of underlying renal anomalies. Data on risk factors of UTI in preterm infants are limited. The aim of this study was to characterize UTI both clinically and microbiologically in premature infants and to define possible risk factors and the role of breastfeeding in its development.

This case-control study was conducted in a tertiary-care neonatal intensive care unit (NICU) between 1995 and 2003.

Intestinal Permeability in Preterm Infants by Feeding Type: Mother’s Milk Versus Formula

Abstract

Background and Objective: Intestinal permeability in preterm infants represents a critical balance between absorption of nutritional agents and protection from dangerous pathogens. This study identified the relationship between feeding type (human milk and formula) and intestinal permeability as measured by lactulose to mannitol ratio in preterm infants in the first postnatal month.

Study Design: Sixty-two preterm (≤32 weeks of gestation) infants had assessment of feeding type and evaluation with enteral lactulose and mannitol administration and urinary measurement at three time points in the first postnatal month.
