Optimizing Support for Breastfeeding as Part of Obstetric Practice

ABSTRACT: Although most women in the United States initiate breastfeeding, more than one half wean earlier than they desire. As reproductive health experts and advocates for women’s health who work in conjunction with other obstetric and pediatric health care providers, obstetrician–gynecologists are uniquely positioned to enable women to achieve their infant feeding goals. The American College of Obstetricians and Gynecologists recommends exclusive breastfeeding for the first 6 months of life, with continued breastfeeding as complementary foods are introduced through the infant’s first year of life, or longer as mutually desired by the woman and her infant.

Guidelines for Diagnostic Imaging During Pregnancy and Lactation

ABSTRACT: Imaging studies are important adjuncts in the diagnostic evaluation of acute and chronic conditions. However, confusion about the safety of these modalities for pregnant and lactating women and their infants often results in unnecessary avoidance of useful diagnostic tests or the unnecessary interruption of breastfeeding. Ultrasonography and magnetic resonance imaging are not associated with risk and are the imaging techniques of choice for the pregnant patient, but they should be used prudently and only when use is expected to answer a relevant clinical question or otherwise provide medical benefit to the patient.

Abstract

The use of computed tomography (CT) and magnetic resonance (MR) imaging has increased tremendously in the past 2 decades. Hence, pregnant and breast-feeding women, although generally healthier than the population at large, are also more likely to require contrast material-enhanced imaging. When a contrast-enhanced CT or MR imaging study is being considered for a pregnant or lactating patient, the potential risks to the fetus related to exposure to radiation, high magnetic fields, or contrast agents must be considered and weighed carefully against the risks of potential misdiagnosis due to withholding contrast agents and imaging studies.

A joint statement of Health Canada, Canadian Paediatric Society, Dietitians of Canada, and Breastfeeding Committee for Canada

This statement by the Infant Feeding Joint Working Group provides health professionals with evidence-informed principles and recommendations. Provinces, territories, and health organizations can use it as a basis for developing practical feeding guidelines for parents and caregivers in Canada.

This statement promotes the communication of accurate and consistent messages on infant nutrition in the first six months. Guidance on nutrition from six months to two years of age will be covered in a separate statement.

Position of the Academy of Nutrition and Dietetics: Promoting and Supporting Breastfeeding

Abstract

It is the position of the Academy of Nutrition and Dietetics that exclusive breastfeeding provides optimal nutrition and health protection for the first 6 months of life, and that breastfeeding with complementary foods from 6 months until at least 12 months of age is the ideal feeding pattern for infants. Breastfeeding is an important public health strategy for improving infant and child morbidity and mortality, improving maternal morbidity, and helping to control health care costs. Research continues to support the positive effects of human milk on infant and maternal health, as it is a living biological fluid with many qualities not replicable by human milk substitutes.

Breastfeeding, Family Physicians Supporting (Position Paper)

Introduction

The American Academy of Family Physicians (AAFP) has long supported breastfeeding. All family physicians, whether or not they provide maternity care, have a unique role in the promotion of breastfeeding. They understand the advantages of family-centered care and are well positioned to provide breastfeeding support in that context. Because they provide comprehensive care to the whole family, family physicians have an opportunity to provide breastfeeding education and support throughout the course of life to all members of the family.

History

Throughout most of history, breastfeeding was the norm, with only a small number of infants not breastfed for a variety of reasons.

A szoptatás az embercsecsemők táplálásának biológiai módja. A testi és lelki egészség számos területére jótékony hatással van, olcsó és környezetbarát. Ezért a szoptatás védelme, népszerűsítése és támogatása olyan cél, amivel a társadalom minden tagja nyer.

A Szoptatás Világhetének céljai 2014-ben

A Szoptatás Világhetét 1992 óta ünnepeljük világszerte augusztus első hetében. A rendezvények célja, hogy valamilyen szoptatáshoz kapcsoló kérdéskörre irányítsák a közvélemény figyelmét. Idei témája arra szeretné felhívni a figyelmet, hogy a Milleniumi Fejlesztési Célok megvalósításában a szoptatás támogatása kulcsszerepet játszhat. A téma időszerűségét az adja, hogy az elérendő célok kitűzött határideje 2015, azaz a következő év.

Óvatosan a bromokriptinnel!

Az EMA Farmakovigilanciai Kockázatértékelő Bizottsága a laktáció befolyásolását szolgáló bromokriptin-kezelés szigorú korlátozását javasolja a szer ritka, de potenciálisan súlyos – több esetben halálos kimenetelű – kardiovaszkuláris, neurológiai és pszichiátriai mellékhatásai miatt.

Júliusi ülésén az Európai Gyógyszerügyi Hivatal (EMA) Farmakovigilanciai Kockázatértékelő Bizottsága (PRAC, Pharmacovigilance Risk Assessment Committee) egyhangúlag foglalt állást a bromokriptin laktációhoz kapcsolódó alkalmazásának szigorú korlátozása mellett. A bizottság a jövőben csak a legindokoltabb esetekben engedélyezné a bromokriptin felírását a tejelválasztás megelőzésére vagy a laktáció leállítására – így a magzat vagy a megszületett gyermek elvesztése esetén vagy a HIV-fertőzött anyák kezelésére. Nyomatékosítják, hogy nem javasolt rutinszerűen alkalmazni a tejtermelés megindulásának kivédésére vagy megszüntetésére, sem a szülés utáni emlőfájdalom és -duzzanat enyhítésére – ilyen esetekben az emlők hűtése mellett, szükség esetén, a paracetamol-tartalmú fájdalomcsillapítók a választandó szerek.

Abstract

In the World Health Organization/United Nations Children’s Fund document Baby-Friendly Hospital Initiative: Revised, Updated and Expanded for Integrated Care, neonatal care is mentioned as 1 area that would benefit from expansion of the original Ten Steps to Successful Breastfeeding. The different situations faced by preterm and sick infants and their mothers, compared to healthy infants and their mothers, necessitate a specific breastfeeding policy for neonatal intensive care and require that health care professionals have knowledge and skills in lactation and breastfeeding support, including provision of antenatal information, that are specific to neonatal care.
